Expert Commentary

This is a sensible combination trial whose logic is sound, since a ketogenic diet and a GLP-1 receptor agonist act through largely different mechanisms, restricting glucose substrate and enhancing fat oxidation on the one hand, augmenting insulin secretion and satiety on the other, so additive metabolic benefit is plausible and is what the trial reports. The inclusion of sleep and quality-of-life outcomes is a nice touch and the improvements there, alongside better glucose, lipids, and insulin sensitivity, paint a coherent picture. I would temper enthusiasm with the practical and methodological caveats the abstract leaves open. Effect magnitudes and adherence detail are limited, six months cannot speak to the well-known sustainability problem of ketogenic eating, and cardiovascular and renal outcomes were not assessed, while ketogenic diets can raise LDL in some patients and are inappropriate in certain kidney disease or eating-disorder contexts. Can I use this with my patients? Yes, selectively and with support. For a motivated patient on a GLP-1 agonist seeking better control, a well-formulated lower-carbohydrate or ketogenic approach is a reasonable adjunct, ideally introduced gradually with dietitian input, lipid monitoring, and attention to electrolytes, while I keep proven therapy and individual tolerability central.
